The Commission has created a new form: Cover Sheet for Action on Claims on Appeal (WCC H-11 AOA, 6/20/08) to facilitate timely processing of appellate court decisions. The form must be accompanied by a True Test copy of the appellate court's order and case docket entry.  If a hearing is required, an Issues form (WCC H-24R) must also be filed for the case to be docketed.  The new form is posted to our Forms & Instructions page.

June 17Our form "Statement of Wage Information" (Average Weekly Wage - 14 Weeks), WCC C-2, 6/09/08 has been revised.
The change affects some of the form's verbiage (text/instructions) for clarification only and does not affect the intent nor function of the form.  As with all WCC forms, the current version must be used.  The revised form is posted to our
Forms & Instructions page.
