The Commission and State Offices are closed Monday, February 19, 2007 in observance of Presidents' Day.  We will open for business at 8 a.m. on Tuesday, February 20, 2007.

February 15Effective immediately, when a carrier or employer intends to terminate temporary total disability payments, WCC Form C-06 "INSURER'S TERMINATION OF TEMPORARY TOTAL DISABILITY BENEFITS" must be utilized. No other version of this form will be accepted. The  *.PDF (Adobe Reader) version of the form is posted on our Forms & Instructions page. (link)

The Commission has revised form H25R (Request for a Hearing for Referral to Maryland Insurance Fraud Division).  Please destroy old versions of this form -- effective immediately the revised form dated 02/09/2007 must be used.  The *.PDF (Adobe Reader) version of the form is posted to our Forms & Instructions page (link).

ATTENTION PRACTITIONERS:  In order to continue to offer efficient and timely service to the Commission's customers, the following procedural guidelines for the rescheduling of matters in which issues have been previously withdrawn will go into effect on February 15, 2007.  The forms discussed below will be available after that date.

  • 1.  Previously withdrawn issues will not be accepted for filing for a period of 90 (ninety) days from the date of withdrawal absent extenuating circumstances.
  • 2.  Where extenuating circumstances exist, the withdrawing party must file WCC Form H-12 "Request for a Hearing on Previously Withdrawn Issues" and include an explanation of the extenuating circumstances and any supporting documentation.  Upon receipt, the request will be forwarded to the Duty Commissioner for action.  The Duty Commissioner's decision will be sent to all parties in the form of a "Notice," so future "Notices" should be checked with care.
  • 3.  Form H-12 will only be accepted for filing once within the 90 (ninety) day period.
  • 4.  WCC Form H-26R "Request for Emergency Hearing" will not be accepted in lieu of Form H-12.

On/about February 15, 2007, this form will be posted to our Forms & Instructions page (link) and will be available in the Online Services subscribers' File Forms menu.

The Commission has revised form H25R (Request for Action on Filed Issues) to include a certification of service that must be completed. Failure to notify opposing counsel prior to the hearing may result in a penalty or fine to be assessed. Please destroy old versions of this form -- effective immediately the revised form dated February 2007 must be used.  The Adobe version of the form is posted to our Forms & Instructions page (link); Online Services subscriber WebForms display only the current WCC form in the File Forms menu.
